Youth Care & Beyond provides individualized support for children and adults with developmental disabilities. Whether someone lives with family, in their own home, or with a Shared Living provider, our goal is to make sure the right support is in place.
We focus on safety, dignity, reliable routines, meaningful relationships, and participation in everyday life.

Opportunities
(adults)
Opportunities supports adults with developmental disabilities who live in their own homes or apartments. Staff provide individualized assistance with personal care, meals, household routines, appointments, transportation, budgeting, community participation, safety, and supp
Best Friends
(children & adults)
Best Friends provides reliable, individualized in-home support for children and adults with developmental disabilities who live with family. Services may include supervision, personal care, daily routines, life skills, community participation, transportation, and other supports identified in the person’s service plan. We work alongside families with respect, consistency, and clear communication.
Transitions Shared Living
(children & adults)
Transitions Shared Living connects eligible youth and adults with trained providers who offer support in their homes. Participants receive individualized supervision, personal care, daily living assistance, transportation, encouragement, and meaningful connection in a family-style setting. The program may serve youth ages 14–19 through CFS and adults with developmental disabilities through DDA.

Services are based on eligibility, funding, assessed needs, provider availability, and the individual’s approved service plan.
