Our Services
01.Opportunities Adult Program
Opportunities supports adults with developmental disabilities who live in their own homes or apartments. Staff provide individualized assistance with personal care, meals, household routines, appointments, transportation, budgeting, community participation, safety, and supported decision-making. The goal is to help each person maintain stable daily living, protect dignity, and have the right level of support in place as needs change.
02.Best Friends
Best Friends provides reliable, individualized in-home support for children and adults with developmental disabilities who live with family. Services may include supervision, personal care, daily routines, life skills, transportation, community participation, and other supports identified in the person’s service plan. We work alongside families with respect, consistency, and clear communication to help daily life feel safer, steadier, and more manageable.
03.Transitions Shared Living
Transitions Shared Living connects eligible youth and adults with trained providers who offer support in their homes. Participants receive individualized supervision, personal care, daily living assistance, transportation, encouragement, and meaningful connection in a family-style setting. The program may serve youth ages 14–19 through Child and Family Services and adults with developmental disabilities through Developmental Disabilities Services.
04.Family Support
Family Support provides practical in-home and community-based assistance for children and families involved with Child and Family Services. Staff help strengthen routines, support parenting, connect families with resources, work toward case plan goals, and respond to everyday challenges with respect and without judgment.
05.Parenting & Visitation Services
Parenting and Visitation Services provide structured support during parenting time and family visits. Services may include supervision, coaching, transportation, documentation, and support with case plan goals. Staff help create safe visits where parents can practice skills and strengthen relationships with their children.
06.Intensive Family Preservation
Intensive Family Preservation provides short-term, high-intensity in-home support when safety or placement is at immediate risk. Services may include safety planning, crisis stabilization, parenting support, counseling, routine-building, and care coordination to reduce risk and help families move toward greater stability.
